We take most Insurance Companies.  You can call our office at 305-532-4478 and find out if
we take yours or contact the Member Service Department phone number on back of your
Insurance card and ask them if our physicians are on your plan.

Deductible, Co-Payment and Co-Insurance is expected at the time services are rendered.
